Ascorbic acid is vitamin C. It is taken to prevent or treat deficiency, and very widely in the hope of preventing or shortening colds — where the evidence is more modest than its reputation suggests. It also has a genuine and underused role in helping the body absorb iron.
What is Ascorbic Acid?
Ascorbic acid and vitamin C are the same thing. It is water-soluble, and — unlike most animals — humans cannot make it. We lost the enzyme for the final synthesis step, which is why it has to come from diet.
It appears in South African products as single-vitamin tablets, effervescent tablets, chewables, powders and injections, and as an ingredient in a great many cold and flu combination products.
In the body, vitamin C does several distinct jobs:
- Collagen synthesis. It is an essential co-factor for the enzymes that stabilise collagen — the structural protein of skin, blood vessels, gums, bone and connective tissue. This is its most important role, and it explains what happens without it.
- Iron absorption. It converts dietary iron into a form far more readily absorbed, and this effect is substantial.
- Antioxidant activity, neutralising free radicals.
- Immune function, supporting several aspects of white blood cell activity.
- Making noradrenaline and carnitine.
Scurvy — the classic deficiency disease — is a collagen failure, which is why it produces bleeding gums, loose teeth, easy bruising, poor wound healing and joint pain. It is uncommon but not extinct, and appears in people with very restricted diets, alcohol dependence, eating disorders and severe poverty.
Being water-soluble, surplus is excreted rather than stored, which is why toxicity is rare — but also why intake has to be regular.
What is Ascorbic Acid commonly used for?
- Preventing and treating vitamin C deficiency and scurvy.
- Colds — by far the most common reason people take it.
- Improving iron absorption, taken alongside an iron supplement or an iron-rich meal. This is one of its most genuinely useful applications, and it is under-used: taking vitamin C with iron meaningfully increases how much you absorb, which matters in iron deficiency anaemia.
- Wound healing and recovery after surgery or injury.
- Supporting immunity generally.
- Skin health, both taken by mouth and applied topically.
- In smokers, who have higher requirements.
- Pregnancy and breastfeeding, where requirements rise modestly.
- Urinary acidification, occasionally.
On colds, the honest position is worth stating. Large reviews have found that regular daily vitamin C does not prevent colds in the general population. It does appear to shorten them slightly — on the order of 8% in adults, roughly half a day on a week-long cold — and taking it only once symptoms start shows no consistent benefit at all.
There is one real exception: people under extreme short-term physical stress — marathon runners, soldiers in subarctic conditions — do show a meaningful reduction in cold incidence.
So it is not useless, but it is not what the marketing suggests. For most people the effect is small, and it comes from taking it regularly rather than reaching for it when you feel a cold coming on.
Who commonly looks for Ascorbic Acid and why
- People with a cold, or trying to prevent one — the largest group by far.
- People taking iron supplements, often on medical advice to improve absorption.
- People with iron deficiency anaemia.
- Smokers, who need more.
- People with restricted or poor diets, including some older adults.
- People recovering from illness, surgery or wounds.
- People with bleeding gums, or easy bruising.
- People interested in skin health.
- People with alcohol dependence, where deficiency is more common.
- People taking high-dose vitamin C for claimed benefits in serious illness — an area where caution is warranted.
What to check on the label before taking Ascorbic Acid
- The amount per tablet. Products range from around 100 mg to 1,000 mg or more. Adults need only around 45 mg daily, and even generous intakes are met by a few hundred milligrams.
- Whether it is effervescent, and if so the sodium content — this is easily overlooked and matters if you have high blood pressure, heart failure or kidney disease. Effervescent vitamin C can carry a surprising amount of sodium.
- Sugar or sweetener content, if you have diabetes.
- Whether it is a combination product. Many cold and flu remedies contain vitamin C plus paracetamol — and if so, you must not take paracetamol from another source.
- Whether you already take a multivitamin containing it.
- The expiry date and storage — vitamin C degrades with heat, light and moisture.
Absorption falls as the dose rises. At 200 mg the body absorbs most of it; at 1,000 mg or more, a decreasing proportion is taken up and the rest is excreted. Splitting a large dose across the day absorbs better than one big one — though for most people the large dose is unnecessary anyway.
Safety, interactions and when to seek professional advice
Vitamin C is among the safer supplements, being water-soluble with surplus excreted. But “safe” is not the same as “no upper limit.”
High doses — generally above about 1,000 to 2,000 mg daily — commonly cause diarrhoea, abdominal cramps, nausea and wind. This is dose-related and settles on reducing.
The more important risk is kidney stones. Excess vitamin C is metabolised partly to oxalate, and high doses increase urinary oxalate — raising the risk of calcium oxalate stones. If you have had kidney stones, or have kidney disease, do not take high-dose vitamin C without medical advice.
Speak to a doctor or pharmacist before taking it if you:
- Have kidney disease or a history of kidney stones.
- Have haemochromatosis or any iron-overload condition, or have had repeated blood transfusions. Vitamin C increases iron absorption, which is exactly what you do not want here.
- Have G6PD deficiency, where very high doses can cause red blood cell breakdown.
- Have diabetes. High doses can interfere with some blood glucose meter readings.
- Are having blood or urine tests, as high doses can interfere with several, including faecal occult blood tests.
- Take warfarin, where very high doses may affect control.
- Are on chemotherapy or radiotherapy. High-dose antioxidants may theoretically interfere with treatments that work by generating free radicals — discuss it with your oncologist rather than deciding alone.
- Are giving it to a child.
A note on high-dose and intravenous vitamin C for serious illness. It is promoted for cancer, sepsis and other conditions. The evidence does not support it as a treatment, intravenous doses reach concentrations impossible by mouth, and it should never replace or delay conventional treatment. If you are considering it, discuss it with the doctor treating you.
See a doctor rather than self-treating if you have:
- Bleeding gums, easy bruising or poor wound healing — these need assessment rather than assumption.
- Persistent fatigue, which has many causes including iron deficiency, thyroid disease and diabetes.
- A cough lasting more than two to three weeks, or fever, breathlessness or unintended weight loss.
- Recurrent infections.
- Blood in the urine, or loin pain — possible kidney stones.
