Cobra 120 is sold as a very high-dose sildenafil tablet for erectile dysfunction. No product of that name is registered in South Africa, and the “120” claims a dose roughly double the maximum ever recommended for licensed sildenafil. That number is the reason for caution.
What is Cobra 120?
A search of the South African Medicine Price Registry returns no product called Cobra 120.
What the register does contain is sildenafil, in dozens of registered products — Viagra, Avigra, Vitrixa, Dynafil, Religra and many generics — all Schedule 4 tablets requiring a prescription, and all at 25 mg, 50 mg or 100 mg.
100 mg is the maximum recommended dose of licensed sildenafil. A tablet claiming 120 mg is above anything that has been through safety testing at that dose.
Two things follow, and both matter more than the marketing.
Sildenafil is a real and effective medicine. It blocks an enzyme called PDE5, allowing blood vessels in the penis to relax and fill in response to arousal. It does not create desire and it does not work without arousal. Prescribed properly, it works well for most men.
A tablet from an unregistered source is not the same thing. Because it has not gone through registration, nothing about it is verified. Products in this market have been found on testing to contain more of the drug than stated, less, none at all, or entirely different substances — including drugs withdrawn from medical use for safety reasons. A number printed on a foil strip is a claim by whoever made it, not a measured fact.
Higher is not better with sildenafil. Above the effective dose, additional drug produces more side effects — headache, flushing, visual disturbance, dangerous drops in blood pressure — without improving the response. If 100 mg is not working, the answer is a medical assessment, not more milligrams.

Why erectile dysfunction is a prescription matter is the part most worth knowing, and it is not bureaucratic caution.
Erectile dysfunction is frequently the first visible sign of cardiovascular disease. The arteries involved are small and narrow early — often years before a heart attack or stroke would announce itself. It is also strongly linked to diabetes, high blood pressure, high cholesterol, low testosterone, depression, sleep apnoea, thyroid disease and the side effects of common medicines.
Treating the symptom with a tablet from an anonymous seller means skipping the assessment that might have found any of those. That is the real cost of buying around the system, and it is larger than the price difference.

What to check before using Cobra 120
The honest advice is: see a doctor or pharmacist instead. Registered generic sildenafil is inexpensive in South Africa, and the consultation is brief and routine.
These are the checks a prescriber makes, and the reason the prescription exists:
- Nitrates are an absolute contraindication. Nitroglycerin, isosorbide mononitrate or dinitrate for angina, or amyl nitrite (“poppers”). Combined with sildenafil they cause a catastrophic drop in blood pressure. This combination kills people, and the risk rises with dose.
- Riociguat for pulmonary hypertension is also absolutely contraindicated.
- Heart disease. Recent heart attack or stroke, unstable angina, uncontrolled blood pressure, significant heart failure or certain arrhythmias — all need assessment, including whether you are fit for sexual activity.
- Alpha blockers for prostate symptoms — doxazosin, tamsulosin — can cause a large blood pressure drop in combination.
- Liver or kidney disease, which affect clearance.
- Eye conditions, particularly retinitis pigmentosa or previous sudden vision loss.
- Sickle cell disease, leukaemia or myeloma, which raise the risk of a prolonged erection.
- Other medicines — some antifungals, antibiotics and HIV medicines substantially raise sildenafil levels, which at 120 mg compounds an already unverified dose.
None of this can be checked by a website that ships tablets.
Possible cautions and when to ask a pharmacist or doctor
Even licensed sildenafil commonly causes headache, flushing, indigestion, a blocked nose, dizziness and altered vision — typically a blue tinge or light sensitivity. All of these become more likely as the dose rises.
Get emergency help immediately for:
- An erection lasting more than four hours (priapism). A genuine emergency — delay causes permanent tissue damage and lasting erectile dysfunction.
- Chest pain during or after sex. Do not take a nitrate to relieve it, and tell the emergency team you have taken sildenafil.
- Sudden loss of vision or hearing, in one or both.
- Fainting, or a severe drop in blood pressure.
- Swelling of the face or throat, or difficulty breathing.
With an unregistered product, add an unknown dose and unknown contents to all of that. If you have taken one and feel unwell, seek care and take the packaging with you — knowing what someone has taken changes what a clinician can do.
