“Alkaline” describes a range of products — alkaline water, alkaline supplements, alkalising powders — sold on the idea that reducing the body’s acidity improves health. The chemistry is real, but the health claim mostly is not: the body controls blood pH within a very narrow range regardless of diet.
What is Alkaline?
“Alkaline” is a description, not a product or an ingredient. It refers to anything with a pH above 7, the opposite of acidic. Products marketed this way include:

The underlying claim needs stating clearly, because it is where most confusion comes from. The “alkaline diet” or “acid-alkaline” theory holds that modern diets make the body too acidic, and that this causes disease — cancer, osteoporosis and fatigue are commonly named — which alkaline products supposedly correct.
The body does not work this way. Blood pH is held between roughly 7.35 and 7.45 by tightly regulated systems involving the lungs, the kidneys and chemical buffers in the blood. This regulation is not optional or fragile — it is fundamental, because enzymes stop working outside that range.
You cannot meaningfully change your blood pH by eating, drinking or supplementing. If blood pH does move outside that range, it is a medical emergency called acidosis or alkalosis, and it happens because of serious illness — kidney failure, uncontrolled diabetes, severe lung disease — not because of diet.
What does change is urine pH. Diet genuinely alters it, and this is where much of the confusion arises: someone tests their urine, sees the pH shift, and concludes their body has been alkalised. What they have actually observed is the kidneys doing their job — excreting surplus acid or alkali to keep blood pH constant. A change in urine pH is evidence the system is working, not that the blood has changed.
What is Alkaline commonly used for?
Products of this type are marketed for:
- General wellness and “detoxification”.
- Reducing acidity or correcting a supposed acid-alkaline imbalance.
- Improving energy and reducing fatigue.
- Bone health, on the theory that acidity leaches calcium from bone.
- Digestive comfort and heartburn.
- Athletic performance and reducing muscle acidity.
- Weight management.
The evidence does not support most of these. Reviews of the alkaline diet have not found that it prevents or treats cancer, and the claim that it does is not supported. The bone health theory has also not held up: studies have not shown that alkaline supplementation meaningfully improves bone density or reduces fractures.
Some narrower uses are genuine, and worth separating from the general claims:
- Sodium bicarbonate is an effective antacid, neutralising stomach acid for short-term heartburn relief. That is straightforward chemistry acting in the stomach, not “alkalising the body”. Products such as Citro-Soda work this way.
- Urinary alkalinisation is a real medical treatment, used to ease discomfort in some urinary tract infections, to prevent certain kidney stones, and in some poisonings — under medical direction.
- Sodium bicarbonate has some evidence in high-intensity athletic performance, at specific doses, though it commonly causes significant gastrointestinal upset.
- Prescribed alkalinising treatment is genuinely used in chronic kidney disease, where the kidneys cannot excrete acid properly. That is a medical situation requiring monitoring.
An alkaline diet rich in fruit and vegetables is often genuinely healthy — but because of the fruit and vegetables, not because of pH.
Who commonly looks for Alkaline and why
- People following the alkaline diet, having read that acidity causes disease.
- People with heartburn or indigestion, who find bicarbonate-based products help.
- People with recurrent urinary discomfort or kidney stones.
- Athletes, seeking a performance edge.
- People with fatigue, hoping to find a dietary explanation.
- People with a cancer diagnosis, or their families — a group who deserve a straight answer: the alkaline diet does not treat cancer, and it should never replace or delay medical treatment.
- People with kidney disease, sometimes prescribed alkalinising treatment legitimately.
- People simply curious about what a product they have been given does.
What to check on the label before taking Alkaline
- The actual ingredients. “Alkaline” tells you nothing on its own.
- The sodium content. This is the most important check. Sodium bicarbonate products can contain substantial sodium, which matters if you have high blood pressure, heart failure or kidney disease, or have been told to limit salt.
- The potassium content, relevant if you have kidney disease or take an ACE inhibitor, ARB or potassium-sparing diuretic.
- Whether it is a medicine or a supplement, and whether it carries a registration number.
- The recommended amount and frequency.
- Whether you already take something similar — antacids, effervescent products and calcium supplements can overlap.
- Sugar or sweetener content, if you have diabetes.
Safety, interactions and when to seek professional advice
These products are not automatically harmless. Regular use of bicarbonate or alkalising salts can cause real problems:
- Sodium overload, raising blood pressure and worsening heart failure — the most common issue.
- Metabolic alkalosis with excessive use, causing muscle twitching, cramps, irritability and confusion.
- Low potassium, which can affect heart rhythm.
- Bloating, wind and abdominal discomfort, since bicarbonate releases carbon dioxide in the stomach.
- Reduced absorption of several medicines. Raising stomach pH changes how some are absorbed — separate other medicines by at least two hours.
Speak to a doctor or pharmacist before regular use if you:
- Have high blood pressure, heart failure or heart disease.
- Have kidney disease, where excreting a sodium or potassium load may not be possible.
- Have liver disease, or any condition causing fluid retention.
- Are on a salt-restricted diet.
- Take diuretics, ACE inhibitors, ARBs, lithium, digoxin, corticosteroids or potassium supplements.
- Take antibiotics, thyroid medicine or iron, whose absorption may be affected.
- Are pregnant or breastfeeding.
- Are giving it to a child.
See a doctor rather than self-treating if you have:
- Persistent heartburn or indigestion, especially over the age of 55, or with difficulty swallowing, unintended weight loss, vomiting blood or black stools.
- Recurrent urinary symptoms or kidney stones, which need proper investigation.
- Persistent fatigue. This has many causes — anaemia, thyroid disease, diabetes, sleep apnoea, depression — none of which are dietary acidity.
- A serious diagnosis such as cancer. Please do not delay or replace treatment with dietary measures. Nutrition genuinely matters alongside treatment, and a dietitian can help with that.
If you regularly need an antacid to feel comfortable, that is worth a conversation with a doctor, since it usually indicates a treatable condition such as reflux.
