Cal-D-Mag describes a combination supplement containing calcium, vitamin D and magnesium. It is taken mainly for bone health, muscle cramps and to make up dietary shortfalls. The name is a description rather than a single registered product, so check your pack for the actual amounts.
What is Cal D Mag?
“Cal-D-Mag” names its three components: Calcium, vitamin D, and Magnesium. Several South African suppliers make products under this or a similar name, and the amounts differ considerably between them. This page cannot tell you what is in your particular pack.
The three are combined because they work together in bone and muscle:
- Calcium is the main structural mineral of bone and teeth, and is also required for muscle contraction, nerve signalling, blood clotting and normal heart rhythm.
- Vitamin D is what allows calcium to be absorbed from the gut. Without adequate vitamin D, calcium intake matters far less — which is the main reason the two are paired. See the calciferol guide for more on vitamin D.
- Magnesium is involved in over 300 enzyme reactions, contributes to bone structure, and opposes calcium in muscle contraction — calcium makes muscle contract, magnesium helps it relax. This balance is why magnesium is associated with cramping. See the Slow-Mag guide.
The body keeps blood calcium within a narrow range, drawing on the skeleton when dietary intake falls short. That is why sustained low intake shows up in bone density rather than in a blood test — and why a normal blood calcium result does not mean your intake is adequate.
Read your label for the elemental content, not just the weight of the salt. A product listing “500 mg calcium carbonate” contains considerably less than 500 mg of actual calcium. Reputable labels state elemental amounts.
What is Cal D Mag commonly used for?
- Supporting bone health, and as part of managing or preventing osteoporosis — the most common reason.
- Muscle cramps, particularly night cramps, where the magnesium is the component of interest.
- Making up a dietary shortfall, where diet supplies too little of one or more.
- Alongside osteoporosis medicines such as alendronic acid, which work poorly without adequate calcium and vitamin D.
- Increased requirements, as in pregnancy and breastfeeding, on professional advice.
- Offsetting the effect of long-term steroids such as prednisone, which reduce bone density.
- Restless legs or muscle twitching, where magnesium is often tried.
A supplement corrects a shortfall; it does not add benefit beyond that. If your diet already supplies enough — dairy, tinned fish with bones, leafy greens, nuts, wholegrains, legumes — supplementing adds little, and more is not better.
On cramps, be realistic. Magnesium is widely taken for cramps and many people report benefit, but the evidence in people who are not deficient is genuinely mixed, particularly for older adults. It is reasonable to try; it is also reasonable to stop if it makes no difference after a few weeks. Cramps that are severe, one-sided, or come with swelling, weakness or numbness need a doctor, not a supplement.
Who commonly looks for Cal D Mag and why
- Postmenopausal women, as falling oestrogen accelerates bone loss.
- Older adults generally, who absorb calcium less efficiently and make less vitamin D in skin.
- People diagnosed with osteoporosis or osteopenia.
- People with night cramps or restless legs.
- People who avoid dairy — through lactose intolerance, allergy or a vegan diet.
- People on long-term corticosteroids.
- People with darker skin, or who cover up or work indoors, who are at higher risk of low vitamin D.
- Pregnant and breastfeeding women, on professional advice.
- People who have had a fracture from a minor fall.
- People with gut conditions affecting absorption, or after bariatric surgery.
- Athletes and physically active people, concerned about cramping.
What to check on the label before taking Cal D Mag
- Elemental calcium per dose — the figure that matters, not the salt weight.
- The vitamin D amount in IU, and whether it is D2 or D3.
- The elemental magnesium amount, and which magnesium salt. Oxide is poorly absorbed and more likely to cause diarrhoea; citrate and glycinate are better absorbed.
- Which calcium salt. Carbonate needs stomach acid, so take it with food — and it is less suitable if you take an acid-reducing medicine such as omeprazole. Citrate absorbs well with or without food.
- How many tablets make up the stated dose.
- Whether you already take anything overlapping — a multivitamin, a separate vitamin D, an antacid containing calcium or magnesium, or a separate magnesium product. Totals add up quietly.
- The expiry date and storage.
Calcium absorbs better in smaller amounts. The body takes up roughly 500 mg of elemental calcium at a time, so splitting the total across the day is more effective than one large dose.
Safety, interactions and when to seek professional advice
These supplements are generally well tolerated. The most common effects are constipation and bloating from calcium, and diarrhoea from magnesium — the two sometimes offset each other, which is one incidental benefit of the combination.
The most practically important issue is interference with other medicines. Calcium and magnesium bind several medicines in the gut and stop them being absorbed. Separate them by at least two hours, and follow specific advice for:
- Antibiotics, particularly tetracyclines such as doxycycline and quinolones such as ciprofloxacin. This can cause treatment to fail outright.
- Thyroid medicine such as levothyroxine.
- Bisphosphonates for osteoporosis, which have strict timing instructions.
- Iron supplements, which compete for absorption.
- Some HIV medicines, including dolutegravir.
Speak to a doctor or pharmacist before starting if you:
- Have kidney disease. This is the key check — impaired kidneys cannot excrete a magnesium or calcium load properly, and magnesium can accumulate to dangerous levels.
- Have a history of kidney stones.
- Have a condition causing raised blood calcium, such as an overactive parathyroid gland or sarcoidosis.
- Take digoxin, where calcium and magnesium changes can be dangerous.
- Take a thiazide diuretic, which reduces calcium excretion.
- Have heart block or a slow heart rate.
- Are pregnant or breastfeeding.
- Are giving it to a child.
- Already take another supplement containing any of the three.
Do not exceed the recommended amount. Excess calcium and vitamin D over time can cause raised blood calcium — with nausea, marked thirst, frequent urination, confusion and constipation — and increase kidney stone risk. Persistent symptoms of that kind while supplementing warrant medical attention.
See a doctor rather than self-treating if you are concerned about bone health, have had a fracture from a minor fall, have persistent or severe cramps, or have muscle weakness, numbness or tingling. A bone density scan and a blood test tell you far more than a guess, and several conditions that mimic a mineral shortfall are treated quite differently.
