B-Co is South African shorthand for vitamin B complex — several B vitamins combined in one product. B-Co injections are widely used, particularly in alcohol dependence and malnutrition, and tablets and syrups are sold over the counter. Formulations differ considerably, so check your pack.
What is B Co?
“B-Co” abbreviates “vitamin B complex” or “vitamin B compound.” It is a naming convention rather than a single registered product, and it is written variously as B-Co, BCo or Bico.
The Medicine Price Registry lists Vitamin B Co injection as a Schedule 3 product. A typical 10 ml ampoule contains:

Injection or tablet? The distinction matters more than it seems:
- Injections bypass the gut entirely, so they work where absorption is poor — in malabsorption, after bariatric surgery, in severe malnutrition, or where a rapid effect is needed. They should be given by a healthcare professional.
- Oral forms are appropriate for most people, and for correcting an ordinary dietary shortfall an injection offers no advantage.
An injection is not a stronger or better version for everyday use. Where the gut works normally, tablets deliver the same nutrients.
Note the amounts in an injection, though — they are substantial. The 50 mg of pyridoxine in a typical ampoule is many times the daily requirement, which is why repeated injections without a clear reason are not a good idea. That is covered below.
What is B Co commonly used for?
- Correcting or preventing a B vitamin deficiency, from poor diet, alcohol dependence, malabsorption or increased requirements.
- Supporting people with alcohol dependence — a major reason B-Co is given in South Africa. Alcohol impairs the absorption, storage and use of several B vitamins, especially thiamine, while often displacing food.
- Severe malnutrition, or recovery from serious illness or surgery.
- Nerve symptoms attributable to a diagnosed deficiency.
- Malabsorption conditions, including coeliac disease and inflammatory bowel disease, or after bariatric surgery.
- Alongside medicines that deplete B vitamins, such as isoniazid for tuberculosis.
- On dialysis, where water-soluble vitamins are lost during treatment.
- In pregnancy, though a dedicated prenatal supplement is usually more appropriate.
It corrects a shortfall rather than adding benefit beyond that. B vitamins are present in meat, fish, eggs, dairy, wholegrains, legumes and leafy greens, and deficiency is uncommon on a varied diet.
It is not an energy supplement. B vitamins help release energy from food, so they are heavily marketed that way — the most common misunderstanding about them. A deficiency causes fatigue, and correcting it resolves that fatigue. Taking B vitamins when your levels are normal does not create energy. Persistent tiredness has many causes — anaemia, thyroid disease, diabetes, sleep apnoea, depression — none of which are fixed by a vitamin you do not lack.

- People with alcohol dependence, or their families, often on medical advice.
- People with poor or restricted diets, including some older adults.
- People with tingling, numbness or burning in the hands or feet.

- People recovering from illness or surgery.
- People seeking more energy, who are usually better served by finding the actual cause.
- People offered a “vitamin injection” privately and wanting to know whether it is worthwhile.
What to check on the label before taking B Co
- Which B vitamins are included, and how much of each.
- The vitamin B6 content specifically. The most important figure on the label, for the reason below.
- Whether B12 and folic acid are included.
- Whether it is oral or injectable.
- Whether you already take anything overlapping — a multivitamin, or a single-vitamin product such as vitamin B6, thiamine or Neurobion. Totals add up quietly, and this is how people reach harmful B6 intakes without realising.
- Any advice on taking it with food.
- The expiry date and storage.
Bright yellow urine after taking a B-complex is normal and harmless — it is riboflavin being excreted, and it indicates surplus rather than a problem.
Safety, interactions and when to seek professional advice
Most B vitamins are water-soluble and surplus is excreted, so they are generally well tolerated. Two exceptions matter:
Vitamin B6 is genuinely harmful in excess. Sustained high intakes cause peripheral neuropathy — numbness, tingling, burning and unsteadiness, typically starting in the feet and hands. These are the very symptoms people often take B vitamins to treat, so someone with tingling who increases their intake can worsen exactly what they are trying to fix. Nerve damage usually improves after stopping, but recovery can be slow and is not always complete. Stop and seek advice if you develop numbness, tingling, burning or unsteadiness — and be particularly careful about repeated injections, where the B6 dose is substantial.
Folic acid can mask a vitamin B12 deficiency, correcting the anaemia while nerve damage continues silently — which is why unexplained anaemia should be investigated rather than self-treated.
High-dose niacin (B3) can cause flushing and itching, and at large doses liver problems.
B vitamins interact with several medicines:
- Levodopa for Parkinson’s disease — B6 can reduce its effectiveness when taken without carbidopa.
- Phenytoin and phenobarbital for epilepsy — B6 can lower their levels and reduce seizure control.
- Isoniazid, hydralazine and penicillamine deplete B6, which is often why it is given alongside them.
- Methotrexate, where folic acid interacts and timing should be medically directed.
Speak to a doctor or pharmacist before starting if you:
- Take any of the medicines above.
- Have existing nerve symptoms, which need diagnosis first — B12 deficiency, diabetes and thyroid disease cause similar symptoms and are treated differently.
- Have anaemia, or are being investigated for it.
- Have kidney disease.
- Are pregnant or breastfeeding.
- Already take a multivitamin or another B product.
- Are giving it to a child.
Thiamine deficiency can be a medical emergency. In someone with alcohol dependence or severe malnutrition, confusion, unsteadiness and abnormal eye movements may indicate Wernicke’s encephalopathy, needing urgent high-dose thiamine by injection — absorption from the gut cannot deliver enough quickly. Untreated it can cause permanent memory damage or death. Treat that as an emergency and get medical help.
